DESCRIPTION:In specialty care\, challenges like overutilization; fraud\, waste\, and abuse (FWA); and wide variations in provider performance drive up costs. Health plans often respond by making blanket reimbursement cuts\, narrowing networks\, and implementing complex utilization management (UM) protocols.
